New Patient Forms and Visit:

In order to streamline your first visit, we kindly request that you fill out your new forms online through your patient portal once it has been activated and set up. Our aim is to gain a comprehensive understanding of your medical history and ensure we deliver the highest level of care. By completing these forms in advance, you assist us greatly in achieving this goal. Should you have any inquiries regarding our patient forms or your appointment, please don't hesitate to reach out to us.

To ensure a smooth first visit, we kindly request that you complete the following steps:

Fill out the New Patient Packet forms online via your patient portal. Separate questionnaires will be available for you to fill out. Please do this before your appointment, preferably at least 24 hours in advance. This allows the doctor sufficient time to review your information and be well-prepared to address your concerns.

Consent forms will be provided and completed during your visit at our office.

Remember to bring a valid government-issued photo ID, your medical insurance cards, and a credit card to keep on file.

It's important to bring your medication bottles with you. This will facilitate a comprehensive medication reconciliation by allowing the medical team to directly observe the actual medication bottles.

Lastly, please wear comfortable shoes and clothing. During your visit, you will undergo a thorough physical examination and assessment.

Please arrive 30 minutes early before your appointment time to allow processing time, or we reserve the right to reschedule your visit.

We are committed to providing high-quality care and ensuring that our services are accessible to all of our patients. In order to maintain the efficiency of our scheduling and to better serve your healthcare needs, we kindly request that all patients provide a credit card to be kept on file.

Please note that we have a policy regarding no-show appointments and late cancellations. A fee of $50.00 will be charged for appointments that are not canceled or rescheduled with less than 24 business hours' notice for established patient, and $100.00 for new patients. For example, if you have a Monday appointment at 2:00 PM, we kindly request that you cancel your appointment no later than 2:00 PM on the Friday before to avoid incurring this fee.
